If your team is using high school laboratory facilities:
- Each high school student must submit a participant consent form signed by their parent or guardian
- Each instructor must submit a participant consent form
- Each team advisor must submit a participant consent form
- A principal of each high school represented on the team must sign the principal consent form
If your team is using university laboratory facilities:
- Each high school student must submit a participant consent form signed by their parent or guardian
- Each instructor must submit a participant consent form
- Each team advisor must submit a participant consent form
- A principal of each high school represented on the team must sign the principal consent form
- The department head in charge of the university laboratory facility must sign the department head consent form

The original hard copy of the consent forms must be sent to iGEM Headquarters at:

Digital copies may be sent to hq (at) igem (dot) org but the team application will not be approved until the original hard copies of the consent form for each team member are received. We recommend shipping the consent forms via a carrier that will provide a tracking number and providing that number to hq (at) igem (dot) org.
