If your team is using high school laboratory facilities"

1. Each high school student must submit a participant consent form signed by their parent or guardian
2. Each instructor must submit a participant consent form
3. Each team advisor must submit a participant consent form
4. A principal of each high school represented on the team must sign the principal consent form

If your team is using university or professional laboratory facilities:

1. Each high school student must submit a participant consent form signed by their parent or guardian
2. Each instructor must submit a participant consent form
3. Each team advisor must submit a participant consent form
4. A principal of each high school represented on the team must sign the principal consent form
5. The lab manager or individual in charge of the laboratory facility must sign the lab manager consent form
